Seeking exceptional PDN nurses (LVNs) to care for adult patients in Katy, Hempstead, League City and Houston. Responsibilities include providing medical treatment in patients' homes, assessing conditions multiple times per shift, and timely documentation in Kantime EMR. Physical Setting: Outpatient Responsibilities:

• Administer medications and treatments as prescribed by the physician.
• Monitor and record patient's condition and notify the physician of any changes.
• Provide personalized patient care, including assistance with daily activities and hygiene.
• Educate patients and their families on proper healthcare procedures and medication usage.
• Collaborate with the healthcare team to develop and adjust patient care plans as needed.

Qualifications:

• 2+ years of experience as a Private Duty Nurse (LVN or RN)
• Current LVN or RN license in the state of Texas
• Strong clinical skills and ability to provide high-quality patient care
• Excellent communication and interpersonal skills
• Ability to work independently and collaboratively with a healthcare team
